Important: If you are insured with Sun Life (including secondary coverage), please do not book with Yanchun Zhang. She is available for other insurance providers. Please ensure all insurance information is complete and accurate before your appointment. Late arrival or incorrect details may result in direct billing being unavailable. In such cases, a receipt will be issued for self-submission. The name of the patient receiving treatment must match the name on the insurance policy. For first-time visits, a valid government-issued ID is required. ✔ Insurance Reminder Coverage varies by plan. You are responsible for any unpaid balance.
